Staff wishing to apply for flexible working in accordance with the School's Flexible Working Procedure, should complete this application form.

Managers who have conducted flexible working meetings in accordance with the School's Procedure, should use this form to formally notify employees of whether their flexible working application has been accepted or rejected.

Staff who wish to appeal against their manager's rejection of their flexible working application, should complete this form.

Appeal Officers should complete this form to notify members of staff of the outcome of their appeal against the rejection of their flexible working application.
